Chronobiology is the study of how biological processes rise and fall on a schedule — and it changes how you should read almost every number in your chart.
Every cell in your body runs on a clock. Your core body temperature, cortisol, melatonin, blood pressure, and even how well your immune system responds to a threat — all of it rises and falls on a roughly 24-hour cycle called your circadian rhythm. In 2017, the Nobel Prize in Physiology or Medicine went to Jeffrey C. Hall, Michael Rosbash, and Michael W. Young for discovering the molecular mechanisms that control this clock. What used to sound like folk wisdom — "get up with the sun," "don't eat right before bed" — turned out to have a precise molecular explanation.
The problem is that almost nothing in conventional healthcare accounts for it. A fasting glucose drawn at 8am and one drawn at 4pm can tell two different stories about the same person. A cortisol level means something different depending on whether it's measured at wake time or midnight. Annual labs capture a single frozen frame; they can't show you the rhythm underneath.

Dr. Goel grew up in a small village in India surrounded by Ayurvedic healing traditions, then trained in critical care and anesthesia in Delhi before completing his internal medicine training at Mount Sinai in New York. His internship began January 2001 — 25+ years in clinical practice and more than 12,000 patients seen, first in conventional internal medicine, now as founder of Prime Vitality Care in San Antonio, Texas, focused on functional medicine, longevity, hormone and peptide therapy, and circadian medicine.
The pattern that led to Time Vitality showed up again and again in his own exam room: patients with "clean" lab panels who were quietly falling apart. He describes his approach as reading "the story your labs can't tell" — because, as he puts it, "your biology is your biography." Time Vitality is that observation turned into a tool: a way to see the pattern in time, not just the number on the page.
Outside the clinic, Dr. Goel is a competitive bodybuilder (IFBB Pro, with a gold medal in Masters Men's Physique and a bronze in the Open division), and is completing Healing the Split, a book built from the same throughline as his newsletter of the same name.
